Liability Waiver I, _______Attendee_________________________________________________ at _____NOVA LLC_______________________________________________________________ ______________________________________________________________________________ HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES being conducted by ______NOVA LLC__________________________________________________ at _______5690 Logan St____________________________________________________________ _____________________________________________________________________________, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I CERTIFY that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. I CERTIFY that there are no health-related reasons or problems which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (1) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: _______NOVA LLC________________________________________________ and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers; (2) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. Attendee I acknowledge that ___NOVA LLC________________________________ and their directors, officers, volunteers, representatives, and agents are NOT responsible for errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf. I acknowledge that this activity may involve a test of a person's physical and mental limits and carries with it the potential for death, serious injury, and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, lack of hydration, and actions of other people, including, but not limited to, participants, volunteers, monitors, and/or producers of the activity. These risks are not only inherent to participants, but are also present for volunteers. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

Terms and Conditions Terms and Conditions: Ticket Holders It is understood that once you are in receipt of a show ticket, the show terms and conditions will be adhered to. Organized by NOVA LLC, subsequently referred to in these rules as the Event Organizers. This ticket is issued under the terms and conditions of the society, venue or land owner where the event takes place. 1. The venue owner or NOVA LLC reserves the right to refuse admission in any reasonable circumstance. 2. The venue owner or NOVA LLC may adopt a bag search on entry or exit if deemed necessary. 3. Tickets are non-refundable. 4. Tickets are non-transferable. 5. NOVA LLC does not permit tickets to be resold for profit or commercial gain. 6. NOVA LLC reserves the right to alter the show content at short notice. In the event of this happening, no refunds will be issued or exchanges offered. 7. This event may be filmed and/or photographed. Attendance at our events affirms your consent to be used as a member of the event audience. 8. NOVA LLC or the venue owners do not accept responsibility for any property lost or damaged at the event and as a result cannot be held liable. 9. The use of video or stills photography equipment for commercial gain is forbidden without the express consent of NOVA LLC. 10. The ticket holder voluntarily assumes all risk and danger incidental to the event whether occurring prior to, during or subsequent to the actual event; including any death, personal injury, loss, damage or liability. 11. NOVA LLC reserves the right to change or amend the date or venue of any event, due to reasons beyond their control. 12. Please note bicycles, scooters and other ATVs are not permitted on the showground during show build up, breakdown and show days. 13. Camera-equipped drones must not be flown at any NOVA LLC events. 14. I recognize that always, an affiliated sponsor, does not profit from ticket sales and the micro dose is 100% free as a gift. Refund Policy 1. This ticket is proof of purchase of the above event and must be presented at the time of admission. 2. This ticket is non-transferable and only one ticket is valid per ticket purchased. 3. Tickets are not to be duplicated for the purpose of falsifying entry. 4. Refunds are available at the discretion of the Event Organizer or Please choose carefully as there are no refunds. 5. The Event Organizer reserves the right to refuse entry. 6. This ticket has been created and managed using the NOVA LLC site and is also subject to the Terms and Conditions of NOVA LLC 7. Proof of age or concession card may be requested. 8. There will be no refunds. 9. Tickets cannot be on-sold by any means at a price greater than the purchase price of the ticket. Scalped tickets may be cancelled, and the ticket holder denied entry. 10. Any persons who are noisy, excessively drunk, threatening or unruly will be asked to leave the premises. 11. The event organizer has all rights to film, photo and video production of this event. 12. If the event is cancelled due to unforeseen circumstances, a full refund will be issued to all patrons. 13. We will not be able to respond to any questions about ticketing or participation during the 24 hours before the start of the event.
